In order to fill a prescription written by a medical professional you must first fill out the patient information and insurance information request form. Then fax your prescription to our office at 954-978-8688. After we receive your information we can start the process of submitting your claim for payment and shipping out your order.
Your insurance coverage using www.Ortholabs.com is no different than if you were to go to a regular Medical office. If there is a problem with your insurance or claim our staff will contact you in order to resolve the matter and get you the product that you need. Some of the items we sell such as Sleep Apnea machines are usually dispensed under the supervision of a doctor by prescription however these items are not controlled by the FDA and can be sold legally without a prescription including replacement parts, masks, hoses, etc. We strongly recommend that you consult a medical professional before using any medical device without proper supervision.
We are located in the United States and are subject to all Federal and State laws and therefore cannot sell any controlled substance or device without the proper paperwork from an authorized and licensed practitioner.
